Ultimate Tournaments

1. Fill out Registration form, on website home page labeled "1- Registration" tab)

You will get an acceptance email and we will hold a spot pending your deposit (10 days)

2.  Send in your teams deposit and Roster to:

Ultimate Tournaments, 477 Dunlay Street, Wood Dale, IL 60191. Email, Fax, or mail a copy of your teams stamped USA Hockey roster.

3. Hotel Reservations: must be made through us. Go to website home page and labeled "2- Hotel Pricing" tab and lookup Hotel Prices and when you find your first choice of accomodations, go to website home page and labeled "3- Book Rooms" and complete the hotel forms and FAX them to 630.206.0617 or email them to any  of the contacts
